First of all, enter the title and description of the form in the "Form details" tab. This information will be used to identify your forms in the back office and in the associated notifications.

You can add a background image if you wish (format 3000 x 3000 pixels at 72 pixels/inch).

This is also where you can configure the thank you message and the email address that will receive notifications of completed forms.

Finally, on the right-hand side of the screen you'll find the sharing feature, where you can associate the form with an establishment and find the link to access the form. The form can be shared by URL or QR Code.
